Department Contact Change Form

Select the role this person plays in the inventory process -
Primary Contact - the person who deals with day-to-day inventory items such as tagging assets, entering them into OASIS or an EIF card, inventories, pending assets, etc.;
Secondary Inventory Contact - the person who backs up or works with the Primary;
Administrator - the next point of contact if the primary contact is unavailable;
Dept Head - Department Head/Director/Dean who has the overall responsibility for the department's assets.
