Student Fiscal Services can prepare a yearly tax form for students who are taking classes at the University of Washington and who are residents or citizens of Canada. Verification of tuition charges, tuition payments, scholarships, and financial aid information are completed for students for the specified tax year.
Please email the Canadian Tax Form TL11A to taxquest@uw.edu and provide your name, student ID number, and year(s) you need filled out. Do not fill out Part 1 of the form.
Alternatively, students can also bring a copy of the form to our office located in Schmitz Hall, room 129 on the 1st floor. We are open Monday to Friday from 12pm to 3pm for office hours.
If you were enrolled in a non-credit class or certificate program offered by the UW Professional Continuing Education (PCE), please contact their accounting office at 206-685-6330 for assistance with Canadian tax forms or email them at CtnmRev@uw.edu.
