Hello,

Linked is the signed 2019 Occupational Safety and Health Administration (OSHA) 300A Summary form for the University of Washington Seattle Campus. Please print and post this document as soon as possible (before February 1 if possible) in locations like a safety bulletin board or a break room where all employees can see it. The 2019 OSHA 300A Summary forms are required by OSHA regulation to be posted from February 1, 2020 through April 30, 2020.

The OSHA 300A Summary form is meant to show important accident statistics for an organization, such as the number of reportable accidents/incidents, the number of cases where employees lost days away from work, the total number of days lost, and the total number of hours worked by employees.

If your department is not located on the Seattle Campus, view and find all OSHA 300A forms for all UW organizations on the EH&S Accident and Injury Reporting website.

OSHA considers accidents/incidents recordable if they result in the following: death, days away from work, restricted work or transfer to another job, medical treatment beyond first aid, loss of consciousness, and significant injuries or illnesses diagnosed by a physician or other licensed health care professional (even if they do not result in death, days away from work, restricted work or job transfer, medical treatment beyond first aid, or loss of consciousness.
