Physical Inventory Response Form

Please use this form to respond to your Physical Inventory Findings letter. By providing the requested information on the form, we will be better able to assist you, and in much more timely manner, than by email. Please fill in *only* the fields that need to be updated for each asset. For tag number changes, please enter the original tag number in the mandatory field, and enter the new tag number in the "Comment/Question" field. If you have more than five assets that need updating, please use the hard copy of your updated inventory to make corrections. Thank you.

Equipment Information Form

EIO will not enter Equipment Information Forms for non-inventorial assets (under $5,000 in total cost) into the OASIS system. It is up to the department to enter, update, and delete these assets if they wish to track them in OASIS. Be aware OASIS is an auditable system, and may not be the best way to track your non-inventorial assets. If you fail to complete the form properly, EIO will return it to the submitter, who will be required to re-submit - so please follow the instructions on the form!

Use this form to request tags for your department. We fill orders in increments of 25.

EIO does not charge for equipment tags.
